Dr. Vandermeer has nearly 30 years of Family Medicine experience, with 22 of those years here in the Seattle area, as a part of the Providence Medical Group. He received his undergraduate degree in Biology at Tulane University before earning his M.D. at Texas Tech University School of Medicine. He then went to Hartford, CT for his internship and residency, at Hartford Hospital and St. Francis and John Dempsey Hospitals respectively.
Dr. Vandermeer believes in personalized, professional care for patients of all ages and feels compassion is what drew him to medicine. He loves the Sherlock Holmes investigative aspect of practicing family medicine. He is married with two sons, and has three generations of extended family here in the Northwest.
Leah Christine Uy, MD has been practicing since 2008. She attended the De La Salle University Health Sciences located in the Philippines and completed her residency at the JFK Medical Center, Family Medicine Program, in Edison, NJ. As a family medicine doctor, Leah strives to provide holistic medical care and ensure continuity of management during periods of illness and as their partner in general wellness.
In her off time Leah enjoys spending time with her family, especially her two children creating arts and crafts, playing in the park. She also loves to sing and read.
Kelvin Wong, MD has been practicing for the last 2008. He attended medical school at De La Salle University Health Science Campus in the Philippines and completed his residency at the JFK Medical Center in Edison, NJ. He believes that being in the primary care setting allows providers to prevent some medical illness from happening, like vaccine preventable illness, modifying lifestyle to prevent diseases like heart attack, stroke and certain cancers. Kelvin's specific values about patient care are to treat everyone how he would want to be treated and how he would want family to be treated as patients.
In his free time Kelvin enjoys staying active with sports and fitness. He enjoys basketball, golf, running, swimming, traveling and spending time with his family. He also enjoys visiting with friends, sports cards and architecture.
Dr. Lee enjoys helping patients make healthy decisions now, in an effort to prevent problems in the future.
Board-certified in internal medicine, he's been in practice since 1999. He graduated from Stanford University with Honors in Biology, and completed his MD at the University of Chicago. His residency was at Mount Auburn Hospital, a Harvard Medical School teaching hospital in Cambridge, MA.
Outside of the office, Dr. Lee enjoys exploring the Pacific Northwest with his wife and two young sons.
Mark values a collaborative approach with patients, and likes to work with them in an effort to help them understand the best evidence-based options for their optimal health and wellbeing.
Mark has been a Physician Assistant since 2011, and has practiced in both Family Medicine and Primary Care since 2013. He graduated from the MEDEX Northwest Physician Assistant training program at the University of Washington.
In his free time, Mark enjoys traveling and outdoor activities with his wife and two sons, including running, hiking, and playing basketball. He has lived in Seattle for almost 25 years.
Phoebe Chitila-James, ARNP began working as a nurse in 2004. She received her Masters in Advanced Practice Nursing with honors and transitioned to Nurse Practitioning in 2014 as a Board Certified Family Practitioner. She believes in the value of providing collaborative, compassionate, evidence based, timely and individualized care. She has enjoyed working in the healthcare sector for the past 15 years. Her experience has spanned from Intensive Care to Emergency room and High acuity urgent care. Phoebe feels most joyful when her patients are empowered, feel their best, and are comfortable with the care they received.
In her free time Phoebe enjoys sports fitness, volunteer work, reading, the arts, strategy games, and gardening.
Alona Habinsky has worked as a nurse practitioner in the Seattle area since 2008, having practiced at Group Health Cooperative and Ideal Image. She holds a Master of Science in nursing, family nurse practitioner tract, from Seattle University. She believes in providing evidence-based, individualized care. In 2008, she co-authored an article that was published in the journal Nursing for Women’s Health.
In her spare time, Alona enjoys swimming, ballroom dancing, traveling, yoga, and reading.
Victoria Lawrence Sanchez started her over 30 year career with a Master's of Science Health-Care Administration from Saint Francis University, Joliet IL. She then furthered her training with a Master's of Science Nursing (FNP), University of South Alabama, Mobile.
She is a compassionate and versatile nurse practitioner. She is skilled at both theoretical and hands-on knowledge of healthcare delivery. She exemplifies expertise with all patient populations, from pediatrics to geriatrics. She is a veteran in the industry … a seasoned provider that partners and collaborates with her patient to achieve the best health care outcome for the individual. Victoria believes that the mind, body and spiritual works as one unit and are essential to wellness. Victoria strives to address these essential components as they relate to healthcare.
She has transitioned her role in medicine as nursing has transitioned over the years. She holds 5-degrees, two of which are Masters Degrees with her final degree, a Doctorate in Nursing Practice that she completed in August of 2015 from Chamberlain University School of Nursing. Victoria's motto is 'never stop learning'.
On a more intimate note Victoria is 21 years married with three children; two sons and one daughter. When she is not busy serving in the role of provider, she demonstrates a great appreciation for the arts. She loves to attend plays, musicals, ballets, and other types of dance performances, and when inspired she writes poetry.
Damon Farrington has worked in the Seattle area as a chiropractor since 2002. He considers himself "hyperlocal," having grown up across the street from Building 28 and attending Interlake HS a mile away. He is a former team chiropractor for the Seattle Sounders FC (2009-2011) and is a former chiropractor for the University of Washington Huskies for all sports. Dr. Farrington graduated with a Bachelor of Science in zoology from the University of Washington in 1998 and graduated summa cum laude from Western States Chiropractic College in Portland in 2002. He has been certified as a strength and conditioning specialist (C.S.C.S.) by the NSCA as well as a Graston and Active Release Technique provider.
Dr. Farrington incorporates the use of soft tissue techniques into his treatments, including Graston Technique, to complement the chiropractic manipulation. Furthermore, Dr. Farrington believes in not just treating the spine, but extremities too. He uses a variety of techniques and believes that by coordinating efforts with a patient's other doctors and/or therapists, he can see less of his patients and they can spend more time doing what they enjoy.
During his time off, Dr. Farrington is an avid runner and reader and enjoys spending time with his wife and kids.
Karen Shepard is a licensed chiropractic practitioner with a background in massage therapy, rehabilitation, and nutrition. She received her training at Palmer College of Chiropractic West and has been practicing in Western Washington since 1998. She believes in a team-based approach to health care and her goal is to assist her patients in reaching their optimum state of health and wellbeing.
Karen won the 2013 and 2014 Top Doctor Award from Seattle Met Magazine. She enjoys the outdoors, yoga, and spends time volunteering for a variety of nonprofits in the community.
Dr. Jason Kim is a Sports Chiropractor and certified strength and conditioning specialist. He brings experience and learning from working alongside a variety of health professionals at the Naval Hospital in Lemoore, CA, and from treating athletes at the 2016 Paralympic Games in Rio, Brazil. Dr. Kim holds an Exercise Science degree from Central Washington University and a Doctor of Chiropractic degree from Palmer College of Chiropractic West.

Craig Carlson is a graduate of The University of St. Augustine for Health Science, where he earned his Master of Physical Therapy. He is certified in manual therapy and he holds a certification as an orthopedic specialist. Having earned his Bachelor of Science from Seattle Pacific University, Craig has practiced his skills at physical therapy clinics throughout Western Washington.
In his free time, Craig enjoys regular workouts, hiking, volunteer work, music, and reading.
Neil Bhagat has a diverse background treating a variety of movement issues in multiple settings since 2003. Having received his training at The Ohio State University, he enjoys enabling patients to take a leading role in the recovery of function. He has a special interest in clinical education and has performed a number of talks on injury prevention for local employers.
Neil enjoys reading, minimalist camping, martial arts, and thoughtful conversations.
Nicole Jones has practiced physical therapy in the Seattle area since 2004. She specializes in outpatient orthopedic physical therapy and has experience in interdisciplinary chronic pain management and occupational rehabilitation. Nicole's treatment style focuses on exercise, manual therapy, neuromuscular reeducation, and postural training. She believes the best success is gained when a patient is a key part of the rehabilitation team. Nicole attended Arkansas State University on a track and field scholarship, graduating with Bachelor of Science and Master of Physical Therapy degrees. Her post-graduate work includes a manual therapy certification from the University of St. Augustine, Florida.
Outside of work Nicole spends her time adventuring with her family, hiking, enjoying various water sports, reading, and traveling.
Mark graduated with a Doctor of Physical Therapy degree from the University of Washington School of Physical Therapy in 2007. Prior to joining Crossover Health, he worked at University of Washington Sports Medicine in Husky Stadium. Mark draws on multiple professional experiences in his practice. He focuses on restoring healthy, functional movement in a collaborative partnership with patients.
Mark enjoys cheering for local sports teams and spending time outdoors in all weather.
Stephanie Yanabu holds a Doctor of Physical Therapy from the University of Puget Sound. She became a board-certified clinical specialist in orthopedic physical therapy in 2013. Her experience includes the treatment of orthopedic injuries such as acute trauma, repetitive overuse, postural dysfunction, sports-specific impairments, and workplace-related injuries. She believes successful rehabilitation is best accomplished with the development of a plan of care unique to each individual's needs.
In her spare time, Stephanie enjoys running, Pilates, yoga, traveling, and spending time with her husband and son.
Denise Maxwell has worked in health management for diverse populations, wellness counseling, motivational interviewing, menu and recipe design, and food product development since 2004. She earned her Bachelor of Science in nutrition and food science from Central Washington University in 2002 and has since earned an Associate of Occupational Studies from Le Cordon Bleu College of Culinary Arts and a dietetic internship professional degree from Oregon Health and Science University. Denise is a registered dietitian nutritionist, a certified dietician in Washington state, a registered health coach II, a certified chronic care professional, and an American Culinary Federation member.
Denise enjoys staying active with hiking, mountain biking, CrossFit, yoga, weight training, and running. She grew up in Washington, is a lifelong Seahawks fan, and enjoys cooking delicious and healthy meals for friends and family.
Natalie Potts has been a registered dietitian with a background in behavioral change strategies and medical nutrition therapy since 2010. Most recently, she served as an onsite health specialist for a multinational aerospace company in Seattle. She coached employees in their personal health and weight management efforts and supported the delivery of wellness services to a vast population of employees.
Natalie has a certificate of training in adult weight management and has developed extensive content for wellness promotion, education, and health presentations. She has also held various positions in community nutrition and campus wellness, organizing health fairs, conducting wellness assessments, and offering educational seminars.
Yi-Jen enjoys working with a wide range of patient populations, and specializes in providing balanced meal coaching for weight control, diabetes, hyperlipidemia, and other chronic disease prevention.
Though she is credentialed by the Commission on Dietetic Registration, and with a BS in Food Science and Human Nutrition from Washington State University, Yi-Jen recently broadened her education to include nutrition as it relates to emotional issues due to neurological imbalances from the brain, such as depression, anxiety, ADHD, dementia, insomnia, and energy deficiency.
Outside of work, she enjoys running, listening to music, cooking and trying new recipes, and spending time with family and friends.
Valerie is onsite at Microsoft in Redmond, WA as a Well-Being and Engagement Specialist for the Be Well program. She is very passionate and excited to be a part of such a wonderful opportunity in building and growing Microsoft's Well-being program.
Prior to her role as Onsite Well-being and Engagement Specialist, Valerie spent over three years working as a Wellness Coach and Wellness Coordinator onsite for Microsoft. She has more than 10 years experience in the fitness and well-being industry. Serving in a variety of roles, including Personal Trainer, Performance Coach, Health and Wellness Coach and Wellness Coordinator.
Valerie is a National Board Certified Health and Wellness Coach. She earned her B.S. in Community Health from Eastern Washington University, where she also studied for a Masters in Exercise Science.
When not working, Valerie enjoys anything active outdoors and indoors including working out, paddle boarding, camping, and being at the lake in the summer, as well as skiing in the winter. She enjoys traveling and spending time with her son and family and friends whenever possible.

Vaishnavi Raghuram is the Lead Optometrist at Living Well Eye Care. Vaish is originally from India and completed her Doctor of Optometry from the New England College of Optometry (NECO), Boston, MA where she was exposed to different models of eye care including Community Health Center, Veterans Affairs, multi-specialty set-up and private practice. At NECO, Vaish was given the 'Ocular Instruments Award' for demonstrating outstanding clinical skills. After graduating, Vaish moved to Tampa, FL where she practiced primary care Optometry for 2 years. She was awarded 'Excellence in Professionalism' and 'Doctor of the year' in 2016.
Vaish has extensive experience in family eye care which includes refractive error correction, contact lens fitting, screening for glaucoma and diabetic retinopathy, management of allergies and dry eyes and co-management for post LASIK care. Valerie Andersson joined the Living Well Health Center pharmacy staff in July 2012. Her professional interests include public health and pharmacy education. Originally from Fresno, California, she received her Doctor of Pharmacy from the University of Washington and her Bachelor of Science in chemistry from the University of Wisconsin-Milwaukee.
In her free time, she volunteers for King County Public Health’s Medical Reserve Corp., volunteers on the MRC’s Autopsy team, volunteers as a Red Cross CPR instructor, and enjoys playing video games.
Alex Kaushansky holds a Doctor of Pharmacy and a Bachelor of Science in biology from the University of Washington. He has worked as a pharmacist since 2004. He has been involved in numerous one-on-one patient sessions and has worked with patients with a variety of conditions and illnesses including hypertension, diabetes, congestive heart failure, and asthma.
An experienced pharmacy manager since 2007, Dr. Ted Gebreselassie holds a bachelor's degree in biochemistry and a Doctor of Pharmacy degree from the University of Washington. As a certified immunizer and specialist in Medication Therapy Management (MTM), Dr. Gebreselassie provides travel health and specialty pharmacy services, as well as medication reconciliation and pharmacy operations support. Outside of the work, he enjoys adventuring with his kids around Washington and Oregon.
After graduating in 2009 from Midwestern College of Pharmacy, Dr. Nora de Souza pursued her specialties in medication adherence, antimicrobial stewardship, and disorders of the pituitary gland. In 2015, she earned her board certification in ambulatory care since 2015. In her spare time, Dr. de Souza enjoys catching up on sleep.
